Applicants name on document must match cardholders registered name (However shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)

Accepted forms of evidence include:

• Completed and Signed Contractor Card

• Card must be current

Issue date = Issue date / Completion date / Training Date

Expiry date = 2 years from issue date

Medical.-.RTAW AOD Statement of Acknowledgement (download document)

Accepted forms of evidence to be completed by Supplier Administrator:

• The Medical & AOD Acknowledgement document OR

• A Completed and Signed Contractor Card & Card which must be current

If the above is not available:

• The Pegasus Workforce Temporary Placeholder will allow Weipa Site Induction and Role to be submitted and training released

• Temporary placeholders will be rejected. Kindly provide the appropriate evidence once it becomes available.

• Certificate showing: Worker full name, Pegasus Worker ID, Session Date, Facilitator/TO name, and approval by Amber Bennett.

• Must originate from L&D QR workflow.

• PDF or clear image.

Issue date = Session Date

Expiry date = 2 years from issue date

Site.Course.RTW Welcome to Country

• Certificate showing: Worker full name, Pegasus Worker ID, Session Date, Facilitator/TO name, and approval by Amber Bennett.

• Must originate from WTC QR workflow.

• PDF or clear image.

Issue date = Session Date

Expiry Date = No Expiry
