• Statement of Attainment must show BCGCM1006A Work safely at heights

• Must be Issued by an RTO with Nationally Recognised Training logo

• Black and white or colour is acceptable

• RTAY Recognition of Prior Competency

• Rio Tinto Record of Completion

• Progress Report

• Theory Assessment or Site Authorisation:

All assessment pages must be included within upload
Matching competency name and code (if with checkbox, this should be completed)
Current licence number and expiry date
Applicant's signature and signed date
Assessor's complete name, signature, and date
Must show that the person is competent and/or authorised to operate

• Issue Date to be entered as date of training completion

• Expiry date to be entered as 2 years from the issue date

• Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)

• Must be 'CTO' training. If 'CTO' is missing from the certification name it is a different training

• A Site Authorisation form showing RTAY LAD-SAF Access System CTO must be uploaded or Flash Tank Access Ladder System Use 

• Site Authorisation form showing Rio Tinto  Yarwun Flash tank Access Ladder system use can be accepted

• All assessment pages must be attached and completed

• Applicant signature, company, and date must be completed

• Assessor signature and date must be completed

• CTO Assessment must reflect that the applicant is competent

• Black and white or color is acceptable

• Scanned document to be clearly legible

NOTE: The Learning & Development section of the form does not need to be completed
