Medical.Competency.Bell Bay Medical Compliance

Medical.Competency. Bell Bay Exemption

• This is a free text field but ideally the medical expiry date will be entered in the following format: DD/MM/YYYY
• This date must align to the date shown on actual medical documentation and must be current but expiry date cannot be more than 2 years in the future. Applications will be returned if a past date is entered.
• Copies of actual medical documentation should not be uploaded / stored in this system
• Exemption can be granted by Bell Bay site administrators

When processing the submitted application, the verification team will use the input expiry date information to assign a site-based competency titled: Medical.Competency.Bell Bay Medical Compliance to the workers profile.

If the word exemption appears in lieu of an expiry date, the verification team will assign a site-based competency titled: Medical.Competency.Bell Bay Medical Exemption to the workers profile.

Rio Tinto site contacts will be asked to verify any exemption requests and may ask to see copies of supporting documentation on site.

Drug and Alcohol Testing.Qualified.Licence/Certificate - Perform drug and alcohol testing

• Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)
• Document must be a Statement of Attainment
• Statement of Attainment must show HLTPAT005 Collect Specimens for Drug of Abuse Tresting (or equivalent)
• Must be Issued by an RTO with Nationally Recognised Training logo
• Black and white or colour is acceptable
• Scanned document to be clearly legible

• Issue Date: Course completion date as shown on document
• Expiry Date: Not Required

• First and last name must match applicant's name in Onsite

Shortened versions of first names are accepted, such as:
"Chris" instead of "Christopher"
"Wil" instead of "William"

Shortened versions of last names are accepted, such as:
"Herrera" instead of "Herrera Trujillo"
"Mohd" instead of "Raza Hasan Mohd"

NOTE:

• If Site Authorisation, the RCC Assessment or CTO Assessment section must be completed

• If Theory Assessment, when RCC is completed, the authorised leader is required to verify with a signature that the applicant is authorised to operate

• The learning and development section of the form does not need to be completed

Issue date = If a certificate has been provided, use the course completion date as issue date. If Theory Assessment or Site Authorisation, use the assessor's signature date.

Expiry date = 2 years from issue date

Medical.Certificate.Medical board of Registration Australia

This registration will advise that they have had their overseas qualifications assessed and have completed the formal process assessment program required to work in Australia

• First and last name must match applicant's name in Onsite

Shortened versions of first names are accepted, such as:
"Chris" instead of "Christopher"
"Wil" instead of "William"

Shortened versions of last names are accepted, such as:
"Herrera" instead of "Herrera Trujillo"
"Mohd" instead of "Raza Hasan Mohd"

• The following evidence can be accepted 
  • Medical Board of Australia Registration

• Must show the type of registration (i.e. Limited, Provisional or General Registration)

• Issue Date: As per document 
• Expiry Date: Not Required

Medical.Bachelor Degree.Medical Studies /Doctor of Medicine

• Applicants name to be shown on documentation
• Applicants name must match (However shortened versions such as 'Chris' for Christopher can be accepted)
• Actual copy of certificate / degree is required. Letters are not accepted in lieu of this.
• Completion date or issue date must be displayed
• Bachelor of Medical Studies/ Doctor of Medicine to be listed
• Document / Certificate / Number

NOTE: Workers must upload an actual degree issued by a university or college demonstrating exactly which course has been completed

NOTE: Higher level qualifications can be used to satisfy lower level competency requirements EG Masters Degree evidence can be accepted for Bachelor Degree competency. Avetta verifiers click here for additional business rule information prior to verification of this competency

Yellow Card

A yellow card must be uploaded (no other medical records or results are to be uploaded)
The Date of birth on the card to match the registered DOB on the system
Must be clear and legible
Must be current

NOTE: old site access cards that have medical expiry date referenced but do not explicitly show they are issued by an actual medical provider are not acceptable

AND

Face-Fit
Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)
Document must indicate that the respirator tested is "Full Face"
Fit Test Report or Sure Fit card are acceptable
Must be current. Note that Fit tests last two years from the test date.

Issue date = The date of the most recently completed 'Spirometry' Component – this will be either the Pre/Post Spirometry date or the Updated Spirometry date.

If the card provided is branded “JOBFIT”, the issue date to reference is the Yellow Card LFT

Expiry date calculations =

Yellow Medical cards expire 6 months from issue date UNLESS an earlier spirometry review is mentioned in the conditions.
Face-Fit Tests expire 2 years from the test date.

The expiration date to be recorded against the competency is whichever of the two expiration dates occurs first.

Yellow Card

A yellow card must be uploaded (no other medical records or results are to be uploaded)
The Date of birth on the card to match the registered DOB on the system

NOTE: old site access cards that have medical expiry date referenced but do not explicitly show they are issued by an actual medical provider are not acceptable

AND

Powered Air Purifying Respirator (PAPR)

 PAPR are permitted for use at BSL only if:

• There is a medical or religious reason preventing the person from wearing a tight-fitting negative pressure respirator; or
• A person cannot successfully pass a respirator face fit approved for negative respiratory protection; or
• Deemed the most appropriate respiratory protection as per risk assessment (i.e. JHA or Level 2 RA)

PAPR must be approved for use on site as per Respiratory Protection HSE Standard

Face-Fit not required for Yellow card if Site Access PAPR approval is completed

Issue date = The date of the most recently completed 'Spirometry' Component – this will be either the Pre/Post Spirometry date or the Updated Spirometry date.

If the card provided is branded “JOBFIT”, the issue date to reference is the Yellow Card LFT

• Must be stamped and signed by the medical practitioner
• Accepted forms of evidence include:
  • Section 4 of the Coal Board Medical
  • Health Surveillance Report
  • Yellow Card

• Must be clear and legible

• Issue Date = Examination date or assessment date
• Expiry Date = 5 years from issue date (unless an indication for a subsequent assessment has been marked and a corresponding date is shown. When applicable, subsequent assessment date should be recorded as expiration date.
