Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)
• Candidate signature & date must be completed
• All assessment pages must be attached and completed
• Assessment result must be ticked
• Assessor name, signature and date must be completed
• Black and white or colour is acceptable
• Scanned document to be clearly legible
• Acceptable replacement for this training must include the following on Recognition of Prior Competency document:
o RTAY C5 Confined Space Awareness (30404390)
o RTAY Confined Space Entry - CTO (30772792)
• "RTAY Confined Space Aware and Standby CTO" is an acceptable replacement for this training

• Issue Date to be entered as date of training completion

• Expiry date to be entered as 5 years from the issue date of statement of attainment

• Must be Issued by an RTO with Nationally Recognised Training logo

• Must be Issued by an RTO with Nationally Recognised Training logo
