EWP Yellow Card (SL)

• Class / Code SL to be listed 

• Must be issued by Elevating Work Platform Association (EWPA)  

• Licence Number must be recorded on card  

• Digital copy is accepted

• Black and white or colour copies of certification is accepted 

• Must be clear and legible 

• Date of completion to be recorded as issue date listed on licence 

• Expiry Date: 5 years from the date of issue or as per evidence if listed

• Must be issued by Telescopic Handler Association of Australia (TSHA)  

• Card Number must be recorded on card  

• Black and white or colour copies of certification is accepted 

• Must be clear and legible 

• Date of completion to be recorded as issue date listed on licence 

• Expiry date to be recorded as expiry date listed on licence 

Familiarisation Training - Drill Rig (Vertical) 

• Applicants name shown on certificate must match the applicants registered name in Onsite
  (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.)

• As this is non-accredited training, the expectation is to upload a statement or certificate for vertical drill rig operation. This could be the equivalent of a similar drilling unit of competency, an internal training program (delivered by a Subject Matter Expert), manufacturer training, or an externally led (i.e RTO) training program in a similar discipline/field.

• Black and white or colour copies of certification is accepted

• Must be clear and legible

• Date of completion to be recorded as issue date listed on certificate/card

• No expiry date is required to be recorded

Logbook - Road-Rail Vehicle 

• Applicants name shown on logbook must match the applicants registered name in Onsite
  (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.)

• All sections must be fully completed

Logbook - TBM 

• Applicants name shown on logbook must match the applicants registered name in Onsite

Product Training - Roadheaders 

• Applicants name shown on certificate must match the applicants registered name in Onsite
  (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.) 

• Statement of Attendance / Completion demonstrating competence in roadheaders 

• Must contain the name of the Training Organisation 

• Black and white or colour copies of certification is accepted 

• Must be clear and legible 

Date of completion to be recorded as issue date listed on certificate 
 

OEM - Batch Plant (Wet) 

• Applicants name shown on certificate must match the applicants registered name in Onsite
  (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.)

• Statement of Attendance / Completion 

• Must contain the name of the Training Organisation 

• Black and white or colour copies of certification is accepted 

• Must be clear and legible 

Date of completion to be recorded as issue date listed on certificate 
 

OEM - Batch Plant (Dry) 

• Applicants name shown on licence/card must match the applicants registered name in Onsite
  (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.)

• Statement of Attendance / Completion 

• Must contain the name of the Training Organisation 

• Black and white or colour copies of certification is accepted 

• Must be clear and legible 

Date of completion to be recorded as issue date listed on certificate 
 

EFNARC Nozzleman Certification Scheme

• Applicants name shown on certificate must match the applicants registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted).

• Certificate must be issued by EFNARC

• Certification of Completion must state Nozzleman Assessment for EFNARC Nozzleman Certification Scheme

• Document / Certificate Number must be recorded on certificate 

• Black and white or colour copies of certification is accepted

• Must be clear and legible

• Date of completion to be recorded as issue date listed on certificate (if specified)

• Expiry date: 5 years from the date of issue. If expiry / valid to date is listed only, this date must be recorded 

• Applicants name on the document must match the applicants registered name in the system (shortened names accepted)

• Evidence provided must be on the GLC approved template – ‘Load Restraint Inspection Assessment

• GLC form with all sections completed must be submitted. Please note the ‘comment’ field is an optional requirement.

• Practical assessment most be listed as competent

• Document must be signed and approved by the site superintendent, safety manager and general superintendent.

• Black and white or colour copies of certification is accepted

• Must be clear and legible

• Issue date to be recorded as the date uploaded in Onsite

• No expiry date is required to be recorded

• Applicants name on the document must match the applicants registered name in the system (shortened names accepted)

• Evidence provided must be on the GLC approved template – ‘Underground VOC – SME Approval Form’

• GLC form with all sections completed must be submitted. Please note the ‘comment’ field is an optional requirement.

• SME approval (underground plant / equipment selected / ticked on the form) must align with the competency selected in the system.

• Document must be signed and approved by the site superintendent, safety manager and general superintendent.
