CPCCOHS1001 - Work safely in construction industry	When Statement of Attainment is uploaded: THIS IS VIEWED AS AN INTERIM CERTIFICATE ONLY Applicants name shown on certificate must match the applicants registered name in Onsite Statement of Attainment issued by a Registered Training Organisation (RTO) to be provided RTO must include Unit of Competency in their scope Unit of competency listed on certificate to match the nominated unit within Onsite. Superseded and equivalent Unit of Competencies are accepted, however must be verified in training.gov.au prior to accepting RTO Name, National Provider Code / RTO Number & Logo and Nationally Recognised Training (NRT) logo to be displayed on document Completion date or issue date must be displayed Document / Certificate Number must be recorded on certificate Accepted evidence types: Statement of Attainment (certificate or card) Academic transcript – results must indicate completion / pass result. USI Transcript with relevant Unit of Competency listed Black and white or colour copies of certification is accepted Must be clear and legible NOTE: RTO registration must only have been current at the time of training completion / awarding the document Cards are accepted in lieu of statement of attainment certificates, providing they show the relevant unit of competency as well as all other necessary information relating to applicant details and RTO details	Date of completion to be recorded as issue date listed on certificate EXPIRY REQUIRED - 60 DAYS FROM ISSUE DATE
White Card Competency Waiver (Internal use only)	A competency waiver is accepted for Gamuda employees only Competency waiver must be completed on Gamuda form with all sections completed Construction Induction (White Card) must be selected as part of declaration Applicants name shown on document must match the applicants registered name in Onsite Competency Waiver must only be uploaded by a Gamuda System Administrator The White Card Competency Waiver must be signed either by: Rebecca Pluijmers; or Carly Allen Black and white or colour copies of certification is accepted Must be clear and legible	Date of completion to be recorded as authorisation date listed on Competency Waiver No expiry date is required to be recorded
Critical Risk Standards	This course is completed via LMS, the certificate is required to be uploaded on completion. If previously undertaken (face to face / virtual) classroom: Attendance list dated and signed by facilitator is required to be uploaded	Date of induction to be recorded as Issue date within Onsite No expiry date is required to be recorded

Mates In Construction - Connector Training	Applicants name shown on certificate must match the applicants registered name in Onsite (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.) Certificate is to be issued by Mates in Construction (MIC) / Safe talk ‘Connector’ or ‘Suicide alertness for everyone’ Training to be listed on certificate Black and white or coloured copy of evidence can be accepted Must be clear and legible	Issue date to recorded as issue date listed on certificate No expiry date is required to be recorded
