  • Applicants name shown on certificate must match the applicants registered name in Onsite
    (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.) 
  • Recognised Training Organisation name to be displayed on document 
  • Certificate to show completion of:  

ISO9001 Quality Management Systems Lead Auditor

OR

RABQSA-QM – Quality Management Systems (Integrated Environment, Health & Safety and Quality Lead Auditor Course)

  • Document / Certificate Number must be recorded on certificate  
  • Black and white or colour copies of certification is accepted 
  • Must be clear and legible 
  • Date of completion to be recorded as issue date listed on certificate 
  • No expiry date is required to be recorded

Shotcrete Slump Testing Record of Training
  • Applicants name shown on document must match the applicants registered name in Onsite
  • Training record is not required to be issued by a Registered Training Organisation (RTO) – internally issued documentation can be accepted
  • Record of Training must include the following:
    • Specify Shotcrete/Concrete Slump Testing and the testing methods
    • Assessor full name, signature, date and qualification/s
    • Competent (C) result
    • Assessment date
  • Black and white or colour copies of certification is accepted
  • Must be clear and legible
  • Date of completion to be recorded as assessment date listed on document
  • No expiry date is required to be recorded

Auditor.Qualified.IRCA Lead Auditor Certification (QMS)
  • Applicants name shown on document must match the applicants registered name in Onsite
  • Certification/card is required to be issued by IRCA
  • Certification to specify Lead Auditor (QMS scheme)
  • Document/certificate number must be recorded on certificate/card  
  • Black and white or colour copies of certification is accepted
  • Must be clear and legible
  • Expiry date: The date of expiration listed on certification/card
