Diver.Licence.ADAS Licence - Diver

• Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)

• Evidence will be ADAS Licence showing 'Diver' as the achieved qualification

• Must be Issued by Australian Diver Accreditation

• Both sides of the licence must be displayed within uploaded file

• Black and white or colour is acceptable

• Scanned document to be clearly legible

• Issue Date: Not required

• Expiry Date: As shown on the licence

Diver.Licence.ADAS Licence - ADAS Licence - Occupational SSBA 2815-2 or 2815-3

• Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)

• Evidence will be ADAS Licence showing 'Diver' as the achieved qualification

• Must be Issued by Australian Diver Accreditation

• Must be Issued by Australian Diver Accreditation

• Both sides of the licence must be displayed within uploaded file
