Radiation Licence	Applicants name shown on licence must match the applicants registered name in Onsite (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.) If the licence only has the first initial of the workers first name, the licence is acceptable Radiation Users Licence to be listed Must be issued by a State Radiological Council or State Environment Protection Authority (EPA) Black and white or colour copies of this document is accepted Must be clear and legible	Date of completion to be recorded as issue date listed on licence Expiry date to be recorded as expiry date listed on licence
