Welder.Statement of Attainment.Polyethylene Butt & Electro Fusion Welding	Name on certificate or card to match person registered (however shortened versions such as 'Chris' for 'Christopher' are acceptable) Accepted Documents: Statement of Attainment Certificate of Attendance Welders Card Competency unit/qualification code must match exactly Must be Issued by an RTO with Nationally Recognised Training logo Black and white or colour is acceptable Scanned document to be clearly legible Note: If a card is supplied there must be a copy of the front and back displaying the units of competency, person's name, issue date as above and must be issued by an RTO.
