The following evidence can be accepted:

• SAP Extract

• Rail Safety Worker Health Assessment:
  • Only a fully completed PART B: HEALTH ASSESSMENT REPORT (page 4) should be uploaded. A full medical will not be accepted. See example column for reference
  • If 'Fit for Duty Subject to Job Modification' is selected, please contact railmedical@riotinto.com (copying in your RTIO Company Representative) for further review

• CMO Medical Review Report must contain all of the following:

  • Contractor’s identification details (First and last name, DOB)

  • Proposed role (Position/Job Role)

  • Category of rail medical completed (Medical type/Assessment type e.g. CAT 1/3) 

  • Date of medical

  • Fitness for work status (Fit for Duty - Conditional or Fit for Duty Subject to Review only accepted)

  • If 'Fit for Duty Subject to Job Modification' is selected, please contact railmedical@riotinto.com (copying in your RTIO Company Representative) for further review

  • Medical expiry date 

  • Sign off by an Authorised Health Practitioner (AHP) or Chief Medical Officer (CMO)

Note: Cat1 Rail Safety Wkr Hlth Assess also acceptable

Issue date = Date of medical

Expiry date = as detailed on evidence received

• If Fit for Duty Subject to Review has been selected, enter the Date of Review as the qualification end Date
• If not expiry date, will need to be rejected
