BOTH 1 & 2 below ARE MANDATED REQUIREMENTS of the role and need to be uploaded into AVETTA as one document for this role:

1. A copy of the required Site Authorisation – Relevant Experience Evidence Form can be downloaded here

 The supplier company will be required to reach out to their site contact directly, providing the above form (with contractor to complete section completed) + evidence of at least 3 years relevant experience for their review.
The authorised Yarwun site contact will then provide a signed copy of the Rio Tinto Site Authorisation Form back to the supplier company.
The supplier company should upload a copy of the completed form WITH all supporting evidence for this requirement and submit for Avetta review who will assess the documentation against the following points:

The site authorisation form approvers name, sign and date fields must be completed for each of the following:
