• Name on card to match the person registered

• Controlled Waste Removal must be displayed

• Must be issued by the relevent Government appointed organisation and the organisation logo must be present

• Licence No & Expiry date to be displayed

• Black and white or colour is acceptable

• Scanned document to be clearly legible

• Issue date to be entered

• Expiry date to be entered as displayed on the card 

• NOTE: Licence renewal is 3 yearly

External.Qualified.EXT060 Passenger Transport Driver Authorisation

• Name on authorisation document to match the person registered

• Document to show 'Passenger Transport Driver'

• Must be issued from WA Govt Department of Transport

• Black and white or colour is acceptable

• Scanned document to be clearly legible

• Issue Date to be entered as date of training completion

• Expiry date to be entered as shown on document
