• Electrical and Instrumentation to be listed

• Other acceptable evidence (but not limited to):

  • Certificate III in Instrumentation

  • UEE31220 Certificate III in Instrumentation and control

  • UEE31211 – Certificate III in Instrumentation and Control

• Black and white or colour acceptable

• Must be clearly scanned and legible

• Issue Date = Date of completion

• Expiry Date = No expiry date to be entered

Electrical.Certificate IV.Electrical - Instrumentation

• Name on certificate to match the person registered

• Electrical and Instrumentation to be listed

• Other acceptable evidence (but not limited to):

  • UEE40411 - Certificate IV in Electrical - Instrumentation

  • UEE40420 - Certificate IV in Electrical - Instrumentation

• Black and white or colour acceptable

• Must be clearly scanned and legible

• Issue Date = Date of completion

• Expiry Date = No expiry date to be entered

Welder.Qualification.Weld Test

• Name on certificate to match the person registered

• A completed weld test must be submitted

• The weld test must be looked over and stamped by head welding supervisor on project. Once stamped and approved, this must be emailed to the relevant site contact to be uploaded to Avetta

• Must be clearly scanned and legible

Tradesperson.Certificate of Competency.Certificate of Proficiency / Apprenticeship Certificate of Completion - Fabrication Trade

• Certificate of Proficiency / Apprenticeship or Certificate III in Engineering - Fabrication Trade to be submitted

• MEM31719 (Or any previous superseded equivalent course codes shown against the mapping of this course on training.gov.au) can be accepted

• Must be clearly scanned and legible

• Completion date - where listed on certificate (this completion date is always used when displayed) OR;

• Issue date - If only an issue date is shown on the evidence, enter this as the issue date
