Radiation Assessing.Licence.NSW EPA Licence - S30 Use radioactive substances for moisture/density determination

• Applicants name shown on licence must match the applicants registered name in Onsite
  (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.)

• Radiation Users Licence to be listed 

• Must be issued by NSW Environment Protection Authority (EPA) 

• Black and white or colour copies of this document is accepted 

• Must be clear and legible 

• Date of completion to be recorded as issue date listed on licence 

• Expiry date to be recorded as expiry date listed on licence
