• The matching competency code needs to be shown

• Certificate must be issued by a registered training organisation (RTO) OR as per example document Service Stream issued Competency to Operate form

• Service Stream CTO Form must have title of the plant, the result COMPETENT must be circled, and include supervisors name and signature

• Service Stream CTO Form must have all three pages uploaded. 

• Issue date = In Onsite should be recorded as the date of completion shown on the certificate

• Expiry date = The expiry date as the date which is calculated to occur 3 years from the issue date
