• Name to match registering person

• Position must be completed

• Contractor Company must be completed

• Must select Primary WHC site

• Induction(s) required – must be completed

• Approval section: "Contract Owner or Coordinator" must be signed

**PAGE 1 - Additional Requirements

• For Trades roles (any roles including ‘Tradesperson’ in the name): The ATUI must include sign off by a person on the authorised approvers list relevant to the role and site. Eg. Electrical roles to be signed by Manager Electrical Engineering, Mechanical Tradesperson roles to be signed off by Mechanical Engineering Manager with the exception of All Open Cut – Other Tradesperson, which can be signed by either the Electrical or Mechanical Engineering Manager relevant to the site.

• All Open Cut – Operator (must be signed off by the Manager of Mining Engineering (MME) or Fixed Plant and CHPP Manager

• Narrabri – All roles require Grant Case's approval signature as the Manager of Mining and Engineering.

• Can be completed at contracting company's leisure; used as reference rather than requirement

PAGE 3 

• Roles that have been selected on the form must match exactly to the portal

• Roles - Where Narrabri Surface (or Underground) is selected as the primary site on page 1 a Narrabri Surface (or Underground) role must be selected.

PAGES 4, 5 & 6

• Complete if applicable  

Form must be no older than 3 months – refer to the latest sign off date in the approvals section.

Issue Date - the latest date in the approval section

Expiry - 3 years from the issue date

Site.Induction.LANDOWNERS SITE ACCESS INDUCTION

Acceptable evidence:

• Must be a Statement of completion - PACS - PROPERTY ACCESS INDUCTION 

• Must show completion of Winchester Downs Property Access Induction

• Name to match the person registered.

• Issue Date: as per the issue date identified on the Statement of completion.

• Expiry Date: two (2) years after the issue date identified on the statement of completion.
