Electrical.Trade.Apprentice Indenture Papers	Name on document to match person Apprentice Indenture papers to be uploaded. Must be issued by Department of Education in relevant state Interim Letter from HVTC which confirms qualification, length of qualification, probation dates, and details of RTO training can be accepted for a period of 90 days if the indenture papers are not yet available	Issue date to be recorded Expiry date 4 years from issue date except in the case the interim letter is supplied where the expiry date will be 90 days from issue date

Training.Statement of Attainment.Provide Pain Management	Applicants name on document must match cardholders registered name (However shortened versions of first names such as 'Chris' for 'Christopher' is acceptable) A Statement of Attainment must be uploaded Must show relevant unit of competency code and name The following competency code can be accepted: PUAEME005A Must be issued by a registered training organisation showing RTO details (RTO Name, Code, and Logo) Must be current	Issue date to be recorded Expiry date is 1 year from issue date
