Dogger.Licence.Dogging - DG High Risk WorkCover

• Name on evidence to match the person registered

• Code which is suitable to the nominated role to be listed - This will be matched to competency selection accordingly

• Black and white or colour acceptable

Acceptable form[s] of Long-term Evidence include:

• Copy of Worksafe issued physical High Risk Work Licence Card - Front side only is mandatory

• Digital High Risk Licence that shows all of the above; Ensuring that Classes & Conditions section needs to be expanded to view all classes.

Acceptable form[s] of Interim Evidence include:

• 'Notice of Assessment' - Accept for a period of 60 days from the completion date

•  High risk licence renewal receipts Accept  for a period of 4 weeks from the receipt date

• Issue and Expiry date to be recorded within Onsite as reflected on the licence unless interim form of evidence has been supplied:

  • HRWL renewal receipts= Expiry date to be recorded 4 weeks from Issue/receipt date.

  • 'Notice of Assessment' = Expiry date will be 60 days from issue date

• NOTE: Occasionally HRWL's will be uploaded with an issue date recorded that is a date in the future. This can be accepted by recording the expiry date only in Onsite with the knowledge that the 'forward date' on the HRWL is a result of continuation from a previous licences end date when renewal is complete
