  • Name on certificate to match the person registered
  • Certificate in Boilermaker Trade or Certificate 3 in Engineering-Fabrication Trade to be listed

  • Black and white or colour acceptable

  • Must be clearly scanned and legible


  • Issue Date = Date of completion
  • Expiry Date = No expiry date to be entered
