Administration.Certificate.DCAC Application Form

• Administrators can accept either the Defence Card or the Defence application (please refer to examples)

• Name on the form to match the name registered in the system

• All fields completed on the application form

• Must state ‘Edinburgh’ location on card

• Front and Back of card required

• Issue Date = Date of file upload

• DCAC form - 60 days 

• DCAC Card - Expiry date that listed on the card

National Police Certificate.Govt Licence/Authority.LL Eng - National Police Check

• Applicants name to be shown on documentation

• Applicants name on document must match name that has been entered into Onsite

• Document must show completion of Police Check / National Criminal History Check

• DCAC pass is also accepted 

• If waiver is uploaded by Lendlease Engineering Team this is to be accepted 

Issue Date = Date shown on the document

If DCAC is uploaded expiry date to be added.
