Accepted forms of evidence include (but are not limited to):

Training Card (only the front is required) or certificate from an RTO or a non-RTO organisation
Training register or letter from the employer company or another organisation identifying Workplace Impairment / Drug & Alcohol Awareness training

First and last name must match applicant's name in Onsite
- Shortened versions of first names are accepted, such as:
  - "Chris" instead of "Christopher"
  - "Niko" instead of "Nicholas"
- Shortened versions of last names are accepted, such as:
  - "Herrera" instead of "Herrera Trujillo"
  - "Mohd" instead of "Raza Hasan Mohd"

Must state "Workplace Impairment" or "Drug & Alcohol Awareness"
Black and white or colour copies are accepted

Issue Date = Date of training completion (if shown on document)
- If completion date is not displayed: the date of upload can be entered as the issue date.

Expiry Date = As per document (only required if shown on document)

For rail projects (other than the Gawler Electrification Rail Project) where this Network requirement is not applicable, a competency waiver will be accepted.

Issue date to be recorded in Onsite as reflected on the Statement of Completion / Attendance Record.
There is a no expiry for this document.
