First Time Upload (Section A only)

·         The applicants name shown on this document needs to match the applicants registered name in Onsite
·         Approval to Supervise Forms should have one of the following codes:
·         - LSB1002F or LLE1002F
·         - LLE1006F
·         All Subcontractor fields (Part A) must be completed for initial upload
·         NOTE: PART A for subcontractors signed by contractor only has 24 hours expiry from the time the Pegasus administrator verifies this document.

Second Time Upload (Fully completed - All Sections) - Lendlease / Joint Venture Approved Document

·         The applicants name shown on this document needs to match the applicants registered name in Onsite
·         Approval to Supervise form with one of the below codes listed
·         - LSB1002F or LLE1002F
·         - LLE1006F
·         All pages of the document need to be included within the uploaded file
·         All Subcontractor fields (Part A) must be completed for initial upload
·         Either black and white or colour copy of document is accepted
·         All questions to be answered using tick boxes in Part B
·         All tick box must be ticked as yes or N/A
·         Document must be signed by Lendlease or Joint Venture representative

Second Time Upload
·         The Issue date in Onsite should be recorded as the date on form
·         No Expiry Date

• The matching competency name needs to be displayed on document

• Must be issued by a registered training organisation (RTO)

• Either black and white or colour copy of document is accepted

•  A relevant trade licence or high risk licence will be accepted in lieu of VOC providing remaining business rule verification points are still met.

  • Non slewing mobile cranes greater than three tonnes capacity (CN)

  • Slewing mobile cranes (C2, C6, C1, C0)

• Must be issued by a registered training organisation (RTO)

• Either black and white or colour copy of document is accepted

•  A relevant trade licence or high risk licence will be accepted in lieu of VOC providing remaining business rule verification points are still met.
