• The applicants name shown on this card needs to match the applicants registered name in Onsite

• The card must show it is refresher training

• The Card must have “Enter and Work in Confined Space”

• Issued date or refresher training date must be on card

• Issue Date = Date of training completion

• Expiry Date = 1 year from the issue date
