• This card has a 3 year expiry.

Rail.Authorised.Aurizon Authorised Person Logbook (6 Months)

• Applicants name must match what is shown in Onsite (However shortened versions of names are accepted EG Chris for Christopher)

• Commencement Date is as per the entry into the Logbook

• This competency must be issued by Aurizon for employees and contractors

• Expiry Date is 6 monthly from the Date of Commencement  (ie. Commencement Date of 1 August 2020 is valid until 1 February 2021), and then each 6 monthly block after (ie 1 February 2021 to 1 August 2021)

• 1 entry minimum must be made in each 6 monthly block.

• Each entry must include the Activity Type.

• Each entry must contain the NP/SNP/AP Name & Number

• Each entry must be signed by the Supervisor name and Signature.

• Issue Date = Commencement Date

• Expiry date = Needs to be calculated using the below sheet. Please ensure you DOWNLOAD the file so that you can use this calculator in excel.

Aurizon Authority to Work Points & Crossing Welder Card

Business Rule – Aurizon – 25/09/2023

• An Aurizon issued Authority to Work Card is required for this competency which must show

  • “Points & Crossing Welder” as the title

  • Person's name

  • Expiry Date

  • Card number

• The competency expires every 3 years (36 months) and must be as recorded as per the card

Aurizon Authority to Work Points & Crossing Restricted Welder Card
