Rail.Course.Aurizon Isolation and Lockout - Isolating Person (Online)

For Contractors:

• Applicants name to be shown

• Applicants name must match what is shown in Onsite (However shortened versions of names are accepted EG Chris for Christopher)

• A certificate or equivalent is required for this competency.

• This competency must be issued by Internally or externally for Aurizon employees and only externally for contractors

• Contractors must provide certificate and signed verification form

For Employees:

• An employee history report or LMS screenshot capturing Isolation and Lockout – Isol Persons is to be uploaded, which is to display qualification name, employee name and expiry date. An attendee qualification report is also accepted for Aurizon employees.

• Employees only require the signed verification form

• Issue date = Date of course completion

• Expiry date = Date which is calculated to be 3 years from issue date

NOTE: The expiry date on the Aurizon LMS or Attendee Qualifications reports could be different to the standard 3 year expiry. In this case, the expiry date that is shown on the report should be accepted

• This competency expires in 3 years

• There is a 3 year duration

Contractors

For Aurizon contractors, evidence for this competency is a copy of: Certificate of Completion for Aurizon Isolation and Lockout – Protected Person (Note we no longer require the foundation certificate to be uploaded)

• The certificate of completion is completed Online and can be found at www.aurizon.com.au

• The certificate of completion will be issued by Learning Seat (But does not show this logo on newly issued certificates)

• Certificate needs to show the corresponding date of completion
  
  

Employees

• For Aurizon employees (must be listed under 'Aurizon' in Onsite), an employee history report or LMS screenshot capturing Isolation and Lockout – Protected Persons is to be uploaded, which is to display qualification name, employee name, service number and expiry date. An attendee qualification report is also accepted for Aurizon employees

• Evidence for this competency is either an employee history report, Aurizon LMS screenshot or certificate of completion.
  
  

For Aurizon employees and contractor’s certificates issued by Col Joy Training services will be accepted prior to the 2^nd of May 2016
