• Applicants name on the document must match name in the system (however shortened versions such as 'Chris' for 'Christopher' are acceptable)

• Accepted form/s of evidence include:

  • Screenprint from Qld Governments Chemical application licence search online portal demonstrating current Licence details

  • Hard copy licence issued by Queensland Government

• Licence number must be displayed on the evidence (Licence Code AXXXXX)

• Issue Date = Date granted (if shown)

• Expiry Date = As shown on licence.  Licence expiry is either 1 or 3 years
