• Must include all pages of the Accreditation including Conditions of Accreditation Certificate must be issued by the National Heavy Vehicle Regulator

• Certificate must show Accreditation Number

• Black and white or colour is accepted.

• Issue Date: Effective date as shown on the evidence

• Expiry Date: Expiry date as shown on the evidence

• Applicants name to be shown

• Applicants name must match what is shown in Onsite (However shortened versions of names are accepted EG Chris for Christopher)

• A certificate or equivalent is required for this competency.

• The competency expires in 3 years

• For Aurizon employees: the competency is either a statement of attainment or a training attendance sheet which can be issued by an internal or external assessor.

• For Contractors: the competency is a statement of attainment which is issued by an external assessor.
  
  NOTE: The newly issued Aurizon Protection Officer cards will eventually replace the statement of attainment and training attendance sheet for both employees and contractors
