Mechanical.Appointed.Mr Arthur South Mechanical Appointment Form

• Must be the Mt Arthur South 'Mechanical' Appointment Form

• Must be form version 6.0

• Name on appointment form to match the person registered

• "Contractor Signature accepting appointments below" must be signed by the worker

• Appointment form does NOT require Thiess signature prior to upload.

• All sections of the form must be complete (section 5 can be incomplete)

• Section 2 for Qualified Mechanical Tradesperson must be filled or Section 3 if they are Non-statutory Mechanical Worker. Both sections if they have multiple roles/appointments.

• Name on appointment form to match the person registered

• Must be form version 1.0

• "Contractor Signature accepting appointments below" must be signed by the worker

• All sections of the form must be complete (however "THIESS Electrical Engineer" section can be left blank)
