• Must state RIIWHS204D or RIIOHS204A or RIIWHS204E

• Evidence must be issued by a registered training organisation (RTO)

• Black and white or colour acceptable

• Must be clearly scanned and legible

• Issue date must be within the last 2 years

• Issue Date = Date of completion

• Expiry Date = 2 years from date of completion
