For Contractors:

Must be a completed and approved version of the Induction Booking Request jotform

• The Job Title listed on the approval form must match the role selected in onsite.

NOTE: If the section of the form showing the following details is not shown on the uploaded form, it would be returned back for amendment because the form MUST show Thiess representative approval:

Approval date must be within 30 days for the form to be accepted

• Issue Date = record the approved date (not submission date)

• Expiry Date = Not required

Generic.Licence.WorkCover Construction Induction White Card

• Name on card to match the applicants registered name in Onsite

• OHS Construction Induction to be listed

• Only the front side of this card is required to be uploaded.

• Thiess will accept the temporary white card evidence; whereby the individual receives a letter / statement (similar to a DL renewal or DL replacement) saying the replacement card will be issued within 60 days.

• To be issued by the relevant / matching state authority:Workcover/ Safework / Worksafe

• Statement of Attainment Accepted.

• Black and white or colour copies accepted

• Issue date: To be recorded in Onsite as shown on the card

• Expiry date: Normally no expiry however 60 day expiry will occasionally be required to be add (only required when a form of temporary evidence has been uploaded).
