From now until January 31st, we are able to accept any document in place of the Emergency Contact form
This rule exclusion will apply until January 31st.
After that time the usual rule will be enforced which means the following template will need to be complete and uploaded:MCD Template.docx

As stated on the form, all sections (across both pages) of the form need to be complete

  • Applicant name on paperwork to match the name of the registered applicant

  • Document must be Issued by a Registered Training Organisation (RTO)

  • Black and white or colour is acceptable

  • Issue date = date of commencement as listed on agreement
  • Expiry date = completion date as listed on agreement
