Identity.Form.MCD - Emergency Contact and Information

From now until January 31^st, we are able to accept any document in place of the Emergency Contact form
This rule exclusion will apply until January 31^st.
After that time the usual rule will be enforced which means the following template will need to be complete and uploaded:

As stated on the form, all sections (across both pages) of the form need to be complete

Issue and expiry date not required to be entered

Training & Assessment.Authorisation Letter.MCD Victorian Based Apprentice/ Trainee Training Contract

• Applicant name on paperwork to match the name of the registered applicant

• Document must be Issued by a Registered Training Organisation (RTO)

• Black and white or colour is acceptable

• Issue date = date of commencement as listed on agreement

• Expiry date = completion date as listed on agreement
