Site.Appointed.Mechanical (Maxwell Surface) Site.Appointed.Mechanical (Maxwell UG)	Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable) A completed Maxwell appointment form for mechanical tradesperson / fitter is to be uploaded A relevant Mechanical trade qualification is to be listed Automotive/plant/heavy equipment will be acceptable Acknowledgment and Appointment sections of the form must be completed showing the name, actual signature, and date Black and white or colour is acceptable	Issue date to be recorded No expiry
