• Must be signed by a Company Representative

• Must be signed by the MACH Energy Package Manager

• Current Drivers Licence MUST be uploaded

• One of the following boxes must be ticked:

  • Driver Familiarisation.Site Drivers Licence – Specified Contractor Work Area / access

  • Operator Authority.Plant and Equipment – Specified Contractor Work Area / access

• If the following boxes are ticked on the form they will NOT be entered by PEGASUS:

  • Driver Familiarisation.Site Drivers Licence – All areas (inc. Mining Operations)

  • Operator Authority.Plant and Equipment – Limited Movement

  • Operator Authority.Plant and Equipment – Open

• If the Nomination Form is submitted with ONLY one of the following boxes ticked it must be returned to the contracting company

  • Driver Familiarisation.Site Drivers Licence – All areas (inc. Mining Operations)

  • Operator Authority.Plant and Equipment – Limited Movement

  • Operator Authority.Plant and Equipment – Open

• Issue Date = as per signed date on the form

• Expiry Date = 2 years from the date of issue

Operator.Authorised.Plant & Equipment – Contractor Work Area

• Must be signed by a Company Representative

• Must be signed by the MACH Energy Package Manager

• Current Drivers Licence MUST be uploaded

• One of the following boxes must be ticked:
