• Name on licence to match the name registered in the system

• Photo to match person

• Date of Birth (DOB) to match applicants DOB in Onsite

• “PTLC” will be the condition shown on the back of a NSW driver’s licence

• Must be current

• Must be issued by NSW state authority

• Issue Date: Not required

• Expiry Date: As shown on the licence
