First Aid.Statement of Attainment.Provide cardiopulmonary resuscitation

• Applicants name to be shown on document

• Applicants name on document must match cardholders registered name (However shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)

• Please note; the certified document that has been issued by the Registered Training Organisation (RTO)** must meet the following requirements to be approved: PLEASE BE AWARE THAT THIS IS NOT THE CASE FOR THE ESSENTIAL ENERGY SPREADSHEET- READ BELOW

•  RTO Name and RTO Code

• **RTO registration must only have been current at the time of training completion / awarding the document

Accepted Unit[s] of competency code and name:

• HLTAID009 - Provide cardiopulmonary resuscitation

• HLTAID001 - Provide cardiopulmonary resuscitation

• Authorised person name or title (does not necessarily need a signature)

• Issuing organisation’s logo / corporate identifier / unique watermark and / or document or certificate number

• Nationally Recognised Training (NRT) Logo (For a Statement of Attainment only - Not Card)

• NOTE:   Accepted also is a spreadsheet from Essential Energy - This will show Essential Energy as the Organisation, Given Name, Surname, Completion date and course details 

• Record the Issue date within Onsite as follows:

  • Completion date / Training Date - if listed on certificate (this date is always used when displayed)

  • Issue date - If only an issue date is listed please enter this as the issue date

• Expiry date to be recorded as 13 months from date of completion

First Aid.Statement of Attainment.Provide First Aid

• Applicants name to be shown on document

• NOTE: Cards are accepted in lieu of statement of attainment certificates providing they show the relevant statement of attainment unit of competency as well as all other necessary information relating to applicant details and RTO details

• NOTE: Temporary / Interim statement of attainment evidence or letters are not accepted.

First Aid. Statement of Attainment. Provide first aid in an ESI Environment

• Applicants name to be displayed on uploaded evidence and to match applicants registered name

• Statement of Attainment (or card showing statement of attainment unit of competency) is required to be uploaded showing the relevant unit.

• The unit of competence should show UETDRRF007 – Provide first aid in an ESI environment.

• Applicants who have completed First Aid. Statement of Attainment – Provide First Aid (see business rules above) can be uploaded as evidence however expiry will be limited to 13 months from day of issue.

• Any Registered Training Organisation (RTO) with this unit of competency on scope can issue the qualification.

• Black and white or coloured copy of the document is accepted

• Record the Issue date within Onsite as follows:

  • Completion date / Training Date - if listed on certificate (this date is always used when displayed)

  • Issue date - If only an issue date is listed please enter this as the issue date

• Expiry date to be recorded as 13 months from date of completion

First Aid.Statement of Attainment.Provide Advanced First Aid

• Applicants name to be displayed on uploaded evidence and to match applicants registered name

• Statement of Attainment (or card showing statement of attainment unit of competency) is required to be uploaded showing the relevant unit.

• The unit of competence should show HLTAID006 or HLTAID014 Provide advanced first aid 

• Any Registered Training Organisation (RTO) with this unit of competency on scope can issue the qualification.

• Black and white or coloured copy of the document is accepted

• The date of completion on the evidence should be recorded as the Issue Date in Onsite

• Expiry date in Onsite should be recorded as the date which is calculated to be 3 years from the Date of training completion

First Aid.Statement of Attainment.Provide advanced resuscitation

• Applicants name to be displayed on uploaded evidence and to match applicants registered name

• Statement of Attainment (or card showing statement of attainment unit of competency) is required to be uploaded showing the relevant unit.

• The unit of competence should show HLTAID007 or HLTAID015 Provide advanced first aid 

• Any Registered Training Organisation (RTO) with this unit of competency on scope can issue the qualification.

• Black and white or coloured copy of the document is accepted

• The date of completion on the evidence should be recorded as the Issue Date in Onsite

• Expiry date in Onsite should be recorded as the date which is calculated to be 1 year from the Date of training completion

First Aid.Statement of Attainment.Provide basic emergency life support

• Applicants name to be displayed on uploaded evidence and to match applicants registered name

• Statement of Attainment (or card showing statement of attainment unit of competency) is required to be uploaded showing the relevant unit

• The unit of competence should show HLTAID010 - Provide basic emergency life support, or any superseded equivalent as detailed at training.gov.au

• Any Registered Training Organisation (RTO) with this unit of competency on scope can issue the qualification

• Black and white or coloured copy of the document is accepted

• The date of completion on the evidence should be recorded as the Issue Date in Onsite

• Expiry date in Onsite should be recorded as the date which is calculated to be 3 years from the Date of training completion

First Aid.Certificate.Mental Health First Aid

• Applicants name to be displayed on uploaded evidence and to match applicants registered name

• Statement of Attainment or certificate (or card showing statement of attainment unit of competency name) is required to be uploaded showing the relevant unit.

• Any Registered Training Organisation (RTO) with this unit of competency on scope can issue the qualification.

• Black and white or coloured copy of the document is accepted

• The date of completion on the evidence should be recorded as the Issue Date in Onsite

• Expiry date in Onsite should be recorded as the date which is calculated to be 3 years from the Date of training completion

Health & Safety.Course.Health & Safety Representative (HSR) - NSW

• Applicants name to be displayed on uploaded evidence and to match applicants registered name

• Evidence must show completion of Health & Safety Representative (HSR) - NSW

• Must be issued by Registered Training Organisation (RTO)

• Black and white or coloured copy of the document is accepted

• The date of completion on the evidence should be recorded as the Issue Date in Onsite

• Applicants name to be shown on documentation

• Applicants name must match (However shortened versions such as 'Chris' for Christopher can be accepted)

• Evidence will be issued by Australian Health Practitioner Regulation Agency (AHPRA) confirming registration

• Document / Certificate / Number must be displayed

• A screenshot from website is also acceptable

• Record the Issue date within Onsite:

  • Completion date - if listed on certificate (this completion date is always used when displayed)

  • Issue date - If only an issue date is listed please enter this as the issue date

• Expiry Date = record if listed on certificate, otherwise, leave blank
