Asbestos Liability Insurance		Business Name and ABN needs to match. If it doesn't match, government certified evidence must be provided to link the ABN or business names together. This is ideally a letter or download from ABR showing the linkage; NOTE: If a letter of linkage is supplied, the documentation will need to be sent to site for their approval prior to verification Minimum amount of 5 million dollars Australian for any one occurrence; Coverage amount to be recorded;If less than 5 million (AUD) the policy is to be returned Insurance must be current Policy must clearly state that Asbestos Works are covered or NOT excluded from the policy	Issue Date = Start of coverage date listed on policy Expiry Date = Coverage end date listed on policy (If the policy is paid on a monthly basis the expiry will be set when the next payment is due) Insurer = Insurer will be listed in policy content Policy Number = as specified on the policy Item = Type of of coverage / policy Amount Description = Same as item Amount = as specified on policy (must be minimum of 5 million AUD or equivalent
Asbestos Removal License - SA	SA	If a Sole Trader or Trust Licence will be in individuals name If a Partnership the name on the Licence to match the Partnership registered If a Company, the name on Licence to match the company registered Asbestos Removal (with relevant class) to be listed Colour, black or white copy required Front and back of licence required when licence is uploaded Must be issued by the relevant states Government appointed organisation logo must be present Must be an SA issued licence	Issue Date = Start date if displayed (if not displayed please leave blank) Expiry Date = As shown on licence
Asbestos Assessor License – SA	SA	This Licence can be accepted in the Company Name OR an Individuals Name Asbestos Assessor License Colour, black or white copy required Front and back of licence required when licence is uploaded Must be issued by the relevant states Government appointed organisation logo must be present Must be an SA issued licence	Issue Date = Start date if displayed (if not displayed please leave blank) Expiry Date = As shown on licence
