Operator.Licence.Backhoe / Front-End Loader (LB)

  • Applicants name to be shown on document

  • Applicants name on document must match cardholders registered name (However shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)

  • Evidence must show qualification to Conduct Backhoe / Front-End Loader (LB) Operations

  • Evidence can be any of the following:

    • A Plant Operator Competency Card with the code LB – Backhoe / Front-End Loader

      • The card must be issued by National Occupational Health & Safety Commission (NOHSC)

      • Both sides of the card must be uploaded

    • Statement of Attainment, Certificate of Completion or card showing the course title

      • Statement of attainment in form of a card is accepted providing that it shows the relevant unit of competency as well as all other necessary information relating to applicant details and RTO details

      • Temporary / Interim statement of attainment evidence or letters indicating they are for temporary issue until statement of attainment certificate is provided to applicant is acceptable for a period of 4 weeks only from the date of the training being complete which should be indicated on the letter providing it has been issued by an RTO and indicates completion of a nationally recognised unit code

      • A Statement of Attainment showing completion of RIIMPO319 (or earlier Superseded units/version of competency are accepted).

      • RTO registration must only have been current at the time of training completion / awarding the document.

  • Record the Issue date within Onsite:

    • Completion date - if listed on certificate (this completion date is always used when displayed)

    • Issue date - If only an issue date is listed please enter this as the issue date

  • No expiry. Unless a temporary / interim statement of attainment evidence or letter is provided which is acceptable for a period of 4 weeks

 

Operator.Licence.Dozer Class LZ

  • Applicants name to be shown on document

  • Applicants name on document must match cardholders registered name (However shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)

  • Evidence must show qualification to Conduct Dozer (LZ) Operations

  • Evidence can be any of the following:

    • A Plant Operator Competency Card with the code LZ – Dozer can be accepted

      • The card must be issued by National Occupational Health & Safety Commission (NOHSC)

      • Both sides of the card must be uploaded

    • Statement of Attainment, Certificate of Completion or card showing the course title

      • Statement of attainment in form of a card is accepted providing that it shows the relevant unit of competency as well as all other necessary information relating to applicant details and RTO details

    • A Plant Operator Competency Card with the code LG or GS – Grader can be accepted

      • The card must be issued by National Occupational Health & Safety Commission (NOHSC)

      • Both sides of the card must be uploaded

    • Statement of Attainment, Certificate of Completion or card showing the course title

      • Statement of attainment in form of a card is accepted providing that it shows the relevant unit of competency as well as all other necessary information relating to applicant details and RTO details

      • Temporary / Interim statement of attainment evidence or letters indicating they are for temporary issue until statement of attainment certificate is provided to applicant is acceptable for a period of 4 weeks only from the date of the training being complete which should be indicated on the letter providing it has been issued by an RTO and indicates completion of a nationally recognised unit code

Operator Elevated Work Platform.Licence.EWPA Yellow Card

  • Applicants name to be displayed on the Licence

  • HPA EWP Yellow card licence to be uploaded

  • Black and white or colour copy accepted.

  • If card is uploaded, both sides are required to be shown within uploaded file

  • Issue date to be recorded in Onsite (if shown)

  • If expiry is shown on licence, this should be reflected in Onsite too
