First Aid.Certificate.Mental Health First Aid

  • Applicants name shown on certificate must match the applicants registered name in Onsite (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.)

  • Certificate stating Mental Health First Aid is required

  • Certificate is to be issued by a Registered Training Organisation (RTO)

  • Completion date or issue date must be displayed

  • Document / Certificate Number must be recorded on certificate

  • Black and white or colour copies of certification is accepted

  • Must be clear and legible

  • Date of completion to be recorded as issue date listed on certificate

  • Expiry Date: 3 years from the date of issue

 

First Aid.Statement of Attainment.HLTAID009 - Provide Cardiopulmonary Resuscitation

  • Applicants name shown on certificate must match the applicants registered name in Onsite (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.)

  • Accepted evidence types:

    • Statement of Attainment (certificate or card)

    • Academic transcript – results must indicate completion / pass result.

  • RTO must include Unit of Competency in their scope

  • Unit of competency listed on certificate to match the nominated unit (HLTAID009) within Onsite. Superseded and equivalent Unit of Competencies are accepted (i.e. HLTAID001), however must be verified in training.gov.au prior to accepting

  • RTO Name, National Provider Code / RTO Number & Logo and Nationally Recognised Training (NRT) logo to be displayed on document

  • Completion date or issue date must be displayed

  • Document / Certificate Number must be recorded on certificate

  • Black and white or colour copies of certification is accepted Must be clear and legible

  • Date of completion to be recorded as issue date listed on certificate

  • Expiry Date: 1 year from the date of issue

  • Accepted evidence types:

    • Statement of Attainment (certificate or card)

    • Academic transcript – results must indicate completion / pass result.

  • Unit of competency listed on certificate to match the nominated unit (HLTAID011) within Onsite. Superseded and equivalent Unit of Competencies are accepted (i.e. HLTAID003), however must be verified in training.gov.au prior to accepting

  • Evidence showing completion of any of the following can also be accepted:

    • HLTFA402 - Apply Advanced First Aid

    • HLTFA311 - Apply First Aid

OH&S-WHS.Course.Lead Investigator Training

  • Applicants name shown on certificate must match the applicants registered name in Onsite (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.)

  • Statement of Attendance OR Statement of Attainment are accepted

  • Evidence must show completion of one of the following:

    • ICAM Lead Investigator Course; or

    • Incident Lead Investigator Training Program; or

    • Conduct safety and health investigations; or

    • BSBWHS505A Investigate WHS Incidents

  • Document / Certificate Number must be recorded on certificate

  • Black and white or colour copies of certification is accepted

  • Must be clear and legible

  • Issue date to recorded as issue date listed on certificate

  • No expiry date is required to be recorded

 

OH&S-WHS.Training.HSR Training

  • Applicants name shown on certificate must match the applicants registered name in Onsite (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.)

  • Certificate is to be issued by a Registered Training Organisation (RTO)

  • Certificate to specify ‘Health & Safety Representative’ Training (5-day duration)

  • Accepted evidence types are:

    • Certificate/Statement of Completion

    • Certificate/Statement of Attendance

  • Document / Certificate Number must be recorded on certificate

  • Black and white or colour copies of certification is accepted

  • Must be clear and legible

  • Issue date to recorded as issue date listed on certificate

  • 1 Year expiry date from issue date
