SMS 4801.Safety Management.AS/NZS4801 OR ISO48001 Certificate OR

ISO45001:2018 

  • Business Name on Certification to match the organisation registered. If it doesn't match, government certified evidence must be provided to link the ABN or business names together. This is ideally a letter or download from ABR showing the linkage.

  • Must be certificated AS/NZS 4801 or OHSAS 18001 or ISO 45001:2018 

  • JAS-ANZ stamp to be present on 4801 - If a certificate is supplied that is a combined 4801 and 18001 there will be NO JAS-ANZ stamp present. This is acceptable.

From March 12, 2018: ISO 45001 will start to be issued in lieu of OHSAS 18001 

From September 30, 2021: OHSAS 18001 will no longer be accepted - ISO 45001 MUST be supplied

  • Issue and expiry as per certification

  • Black and white or colour is acceptable

  • Issue and expiry as per certification

  • All fields must be completed

  • The sub-contractor company name must match the registering company in the system.

  • The Authorised Company Representative from the Principal Contractor, must complete the form and sign and date

  • The Authorised Company Representative from the sub-contractor company, must sign and date

The following Sedgman personnel are able to approve: 

  • Christopher Forbes

  • Bradley Mcintyre

  • Patrick Jacobsen

  • Issue date – Date signed by principle company

  • Expiry date – 5 years from issue date

 

Subcontractor Information Form

  • All fields must be completed.

  • The primary contractor company name must match a registered company in the system.

  • The sub-contractor company name must match the registering company in the system.

  • The Authorised Company Representative from the Principal Contractor must print name and sign.

  • The Authorised Company Representative from the sub-contractor company must print name and sign.

  • The Principle Contractor Representative must circle the relevant company, print name and sign.

  • A Secondary Principle Contractor is not mandatory

  • Principle Contractor signatures do not need to be verified

  • Issue date – date of verification

  • No expiry date

 

Sub-Contractor Details

  • At least one line must be completed.

  • The respondent must print their name, sign and date the form.
