• MDG25 Awareness Training or TRG Hot Works (Cutting and Welding) should be shown on the document

• Issue date must be within the last 5 years

• Black and white or colour is acceptable

• Issue date = Date of completion

• Expiry Date = To be recorded 5 years from date of Completion
