Note: LOA in this instance = Letter of Authorisation not Letter of Appointment

Document must be the "Competence Verification & Authorisation – Driller Roles" (for contractors, always use the latest form available at http://nswecinductions.com.au/the-registration-process/

• VOC Type must match role selected in the portal – Note - Please ignore “Return to Contractor for Upload in Pegasus” – this is for internal use only.

• Pegasus Number must match applicant's Onsite ID

• Operator Name must match applicant's name in Onsite (although shortened versions such as "Chris" instead of "Christopher" are acceptable)

• Issue Date =Date of approval by Mine Engineering Manager

• Expiry Date =2 years from Issue Date
