Applicants name to be shown on documentation
Applicants name must match (However shortened versions such as 'Chris' for Christopher can be accepted)
Must state the workers name, company, DOB and signature to ensure accurate.
Required as evidence that a Mines Rescue Induction has been completed – MUST be current
Front and back required

Mines rescue attendance sheet also accepted. The sheet must include the following;
Must be on Mines rescue template
Must be current within 12 months
Must state in the course name must include self-rescuer/self-escape training

'Sedgman training assessments can be accepted in place of the Mines Rescue Induction/Attendee Sheet'.
 

Issue Date = Self Escape issue date
 Expiry Date = 12 months from date of issue
