• Business Name and ABN needs to match. If it doesn't match, government certified evidence must be provided to link the ABN or business names together. This is ideally a letter or download from ABR showing the linkage or a Trust Deed

• We can accept ‘PTY LTD’ or ‘(Pty) Ltd’  in lieu of ‘Proprietary Limited’ when this is the only difference in the company name between what’s shown on company compliance paperwork verse what’s shown in onsite.

New Zealand Companies
Must be issued by New Zealand Companies Office.

Certificate of company registration from relevant government body.

Income Protection Insurance

• Whilst not a legal requirement, it is good practice for sole traders to have income protection, and it is an Otis requirement for Australian and New Zealand sole traders

• Income protection is in a individual name therefore No ABN will be on the document.

• We can accept ‘PTY LTD’ or ‘(Pty) Ltd’  in lieu of ‘Proprietary Limited’ when this is the only difference in the company name between what’s shown on company compliance paperwork verse what’s shown in onsite.

• There is no minimum amount for Income Protection and the amount does not need to be recorded

• Must be current

• Must specify the state in which the certification has been uploaded for

• Must specify Income Protection or Accident and Sickness

NOTE: If the policy is paid on a monthly basis, the document must show when the recent payment was made or when the next payment is due

• Issue Date = Start of coverage date listed on policy

• Expiry Date = Coverage end date listed on policy (If the policy is paid on a monthly basis the expiry will be set when the next payment is due)

• Insurer = Insurer will be listed in policy content

• Policy Number = as specified on the policy

• Item = Income Protection or Accident and Sickness as specified on the policy

• Limit of liability is a minimum of $10 AU million for Australia.

• Otis will accept special and general liability insurance as long as it mentions that it covers Public Liability insurance within the policy

• If the amount shown on the document is not listed as an Australian Dollar amount (e.g. US or EURO), use a currency converter to check the amount - if the conversion at the verification processing time meets the required Australian Dollar amount the document can be approved.  In the Comment Field please enter the confirmation that the currency converter process has been completed e.g. ‘Currency Converted and meets the AUD minimum amount required’.

NOTE: The type of policy can be listed in the 'liability limit' section of the policy (e.g. under the class or description it may refer to a policy type that 'requires approval' however under the limit or interest insured section if it refers specifically to an accepted type listed above this would be deemed acceptable.

NOTE: If the policy is paid on a monthly basis, the document must show when the recent payment was made or when the next payment is due

• Period of Insurance dates (start and end) to be recorded as the issue and expiry dates in the system

• Issue Date = Start of coverage date listed on policy

• Must be a current New Zealand Public / Product Liability document

• Limit of liability is a minimum of $5 NZD million

• Otis will accept special and general liability insurance as long as it mentions that it covers Public Liability insurance within the policy

• Must match the company name in the system

• The NZBN (New Zealand Business Number) must match

NOTE: The type of policy can be listed in the 'liability limit' section of the policy (e.g. under the class or description it may refer to a policy type that 'requires approval' however under the limit or interest insured section if it refers specifically to an accepted type listed above this would be deemed acceptable.

NOTE: If the policy is paid on a monthly basis, the document must show when the recent payment was made or when the next payment is due

• Must be a current Fijian Public / Product Liability document

• Limit of liability is a minimum of $5 FJD Million 

• Otis will accept special and general liability insurance as long as it mentions that it covers Public Liability insurance within the policy

• Must match the company name in the system

• The Fiji Company Registration / Tax Number

NOTE: The type of policy can be listed in the 'liability limit' section of the policy (e.g. under the class or description it may refer to a policy type that 'requires approval' however under the limit or interest insured section if it refers specifically to an accepted type listed above this would be deemed acceptable.

• Limit of liability is a minimum of $432 VUV Million  ($5M AUD)

• Otis will accept special and general liability insurance as long as it mentions that it covers Public Liability insurance within the policy

• Must match the company name in the system

• The Vanuatu Company Registration / Tax Number

NOTE: The type of policy can be listed in the 'liability limit' section of the policy (e.g. under the class or description it may refer to a policy type that 'requires approval' however under the limit or interest insured section if it refers specifically to an accepted type listed above this would be deemed acceptable.

NOTE: If the policy is paid on a monthly basis, the document must show when the recent payment was made or when the next payment is due

• We can accept ‘PTY LTD’ or ‘(Pty) Ltd’  in lieu of ‘Proprietary Limited’ when this is the only difference in the company name between what’s shown on company compliance paperwork verse what’s shown in onsite.

• The ABN must match

• Consulting includes professional technical or design services and typically in writing. (Example: Engineering design, drafting, and inspection and testing)

• Certificate of currency for accident insurance with the following details:

• Name of insurer

• Policy Number

• Expiry Date

• Sum Insured

• Restrictions

• Look for validity dates. If the premium is paid monthly, the certificate of currency is only valid for that month. They will need to update monthly.

• Company name and ABN must match the details registered in Onsite Track Easy.

• Issue and Expiration as per certificate of currency

• Certificate of Currency for ACC – Primary Workers compensation insurance

• Name of insurer

• Policy Number

• Expiry date

• Company name and ABN must match the details registered in Onsite Track Easy.

Following this date all companies will be issued the new version of SA Workers comp which will show a 1-year duration and be called ‘Certificate of Registration’.

• Certificate of Currency for ACC – Secondary Workers compensation insurance
