- Business Name on Certification to match the organisation registered. If it doesn't match, government certified evidence must be provided to link the ABN or NZBN or business names together. This is ideally a letter or download from ABR or NZBN Office showing the linkage.

- Must be certificated AS/NZS 4801 or ISO/OHSAS 18001

- JAS-ANZ stamp to be present on 4801

- If a certificate is supplied that is a combined 4801 and 18001 there will be NO JAS-ANZ stamp present which is acceptable.

- OHSAS 18001 is International and therefore must be issued outside of Australia

From March 12, 2018: ISO 45001 will start to be issued in lieu of OHSAS 18001 

From September 30, 2021: OHSAS 18001 will no longer be accepted - ISO 45001 MUST be supplied

- Black and white or colour is acceptable

• Issue Date = Original certification date
• Expiry Date = As listed on evidence

• Black and white or colour is acceptable

ISO 9001 Certificate

• Must be certificated ISO 9001 Certificate
• Black and white or colour is acceptable
