EWP Yellow Card (SL)

·         Applicants name shown on licence/card must match the applicants registered name in Onsite

·         Class / Code SL to be listed

·         Must be issued by Elevating Work Platform Association (EWPA)

·         Licence Number must be recorded on card

·         Black and white or colour copies of certification is accepted

·         Must be clear and legible

·         Date of completion to be recorded as issue date listed on licence

·         Expiry Date: 5 years from the date of issue or as per evidence if listed

Telescopic Handlers Association Australia (TSHA) Gold Card

·         Applicants name shown on licence/card must match the applicants registered name in Onsite

·         Must be issued by Telescopic Handler Association of Australia (TSHA)

·         Card Number must be recorded on card

·         Black and white or colour copies of certification is accepted

·         Must be clear and legible

·         Date of completion to be recorded as issue date listed on licence

·         Expiry date to be recorded as expiry date listed on licence

Operator.VOC.Apply Shotcrete Underground - Internal Verification of Competency (VOC)

·         Applicants name shown on document to match the applicants registered name in Onsite (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.) 

·         VOC to be issued by Tunnelling Solutions  

·         The competency must be indicated on the Verification of Competency (VOC) and align with the nominated competency within Onsite

·         Must be signed and dated by the assessor

·         Worker must show as competent

·         Date of assessment to be recorded as Issue date within Onsite

·         2-year expiration date from date of assessment to be calculated as expiry date and recorded within Onsite

·         Applicants name shown on certificate/card must match the applicants registered name in Onsite (However shortened versions of first names such as 'Chris' for 'Christopher' OR ‘Dave’ for ‘John Dave’ are accepted.) 

·         A Temporary Validation Checklist can be accepted as an interim document

·         3^rd party Verification of Competency (VOC), issued by a Registered Training Organisation (RTO) can only be accepted  

·         The competency must be indicated on the Verification of Competency (VOC) and align to the nominated competency within Onsite.  

·         RTO must have the relevant Unit of Competency in their scope 

Forms of Evidence that are NOT ACCEPTED: 

·         High Risk Licences  

·         Plant Operator Tickets 

·         Contracting company issued internal VOC (where issuing company is not a Registered Training Organisation) 

·         Statement of Attainment issued training
