• Expiry date in Onsite should be recorded as the date which is calculated to be 3 years from the Date of training completion

First Aid.Statement of Attainment.CYP - HLTAID010 - Provide Basic Emergency Life Support

• Applicants name to be displayed on uploaded evidence and to match applicants registered name

• Statement of Attainment (or card showing statement of attainment unit of competency) is required to be uploaded showing the relevant unit.

• The unit of competence should show Provide Basic Emergency Life Support

Site Underground Mandatory Fit Test

• Name on test results to match the person registered (However shortened first names such as 'Chris' for 'Christopher' are accepted)

• Overall Fitness must be a pass to accept. 

• Signature is not required 

• Document with Qualitative face fit testing listed accepted 

• Can be issued by a company 

• Issue = Test date on form 

• Expiry = 3 year from issue date
