• The applicants name shown on this statement of attainment needs to match the applicants registered name in Onsite

• The matching competency code needs to be shown or RIIOHS204A, CPCPCM2055A or RIIWHS204D or RIIWHS204E

• Statement of Attainment must be issued by a registered training organisation (RTO) This may be either card/ticket o SOA certificate

• A verification of competency (VOC) can be accepted in lieu of SOA

Also ACCEPTED (interim certs)

• A letter issued from Holmesglen the course code is to match that of the competency 

• Attendance record for the listed course from RTO

• The expiry date in Onsite should be recorded as:

  • Non-Interim Evidence Expiry Date = 2 years from the date of training completion
