10 Year Major inspection certification	NOTE: Major Inspection records are also required to be stored with plant item on site Completed by a competent person: Professional engineer NDT technician Qualified tradesman Date of inspection at least 10 years from the date the crane was first commissioned/ registered or reregistered whichever was first. No non-compliances	Issue/inspection Date = Record this as it is listed on the recertification Expiry Date = recertification date + 10 years

Access equipment - MEWP inspection	For all MEWP both < and > 11 metres: Report to be on company letterhead Company completing report to provide details including: ABN Contact including address and phone Report to be current – ie date on report within 12 months of current date For MEWP > 11 metres: Copy of user SafeWork NSW (or equivalent) high risk work licence Copy of plant SafeWork NSW (or equivalent) high risk work registration	Issue/inspection Date = Record this as it is listed on the certificate Expiry Date = Record this as issue date + 12 months.

EM Plant used to lift freely suspended loads - Verification of lift rating chart, anti hose burst protection and rated lift point.
