• Statement of Attainment in .PUASAR025A - Undertake Confined Space Rescue to be listed

• Issued by an RTO / Nationally Recognised Training logo

• No expiry

• Black and white or colour acceptable

•  Issue date to be recorded

• Name on certificate to match the person registered

• Document must be a Statement of Attainment

• Statement of Attainment in Work Safely at Heights (RIIWHS204) to be listed

• Competency unit must match exactly with the exception of the version

• Must be Issued by an RTO with Nationally Recognised Training logo

• Black and white or colour is acceptable

• Scanned document to be clearly legible
